1372. Unprecedented West Nile Virus Outbreak — Maricopa County, Arizona, 2021

Abstract Background West Nile virus (WNV) causes a mosquito-borne disease that can lead to permanent paralysis or death. WNV became endemic in Maricopa County 2003. The Department of Public Health (MCDPH) and Environmental Services Vector Control (MCESD VC) Arizona (ADHS) collaborated monitor respond mosquito surveillance human cases. We describe an unprecedented outbreak 2021. Methods MCESD VC monitored vector complaints, positive traps, index (VI) data. Reported cases (using the CSTE/CDC case definition) were interviewed by MCDPH. MCDPH ADHS sent alerts healthcare providers regarding symptomology clinical testing guidance. consulted with and/or Infection Preventionists during investigations. Molecular analysis of West Nile virus (WNV) isolates obtained during a 2010 outbreak in Maricopa County, Arizona, USA, demonstrated co-circulation of 3 distinct genetic variants, including strains with novel envelope protein mutations. These results highlight the continuing evolution of WNV in North America and the current complexity of WNV dispersal and transmission.
